People saying "How are you" when you first meet socially, are just really saying the polite thing.
They are just saying hello.
You could say anything including "I am great" and really mean nothing.
If a person is asking genuinely, "how are you?"
I am great can be a rebuff to their enquiry or a statement of fact.

@lonelyloner In a casual conversation, any reply goes. It really gives no idea how a person is going. SOme say it just to pretend everything is going wonderfully when indeed things are not. Others say it as a habit. Some probably mean it.
