Normal or perverted? Depends on what one means by these words.
The more important question is: Is it harmful? And it 100% isn't. There's absolutely nothing wrong with what's going on in that picture.

So, should they come out?
Do you mean come out as gay? Crossdressing doesn't necessarily mean that a person is gay. And even if, there are good reasons not to come out. There is no should or shouldn't. There are equally valid reasons to do it and not to do it. Of course it's always generally better to be true to oneself, but individuals can have good reasons to remain in the closet.
