Anyone can hire a guard. I don't see a difference between the guys Elon Musk has guarding him and the guy at the local Sam's Club who's there to keep people from running out without paying. I'm just not sure what the point is of taxing them at $100 per man-hour. Do you tax the first but not the second? I'm sure Musk could set up a company that does nothing but hire his bodyguards. It would be legally indistinguishable from the Sam's Club guard.
And the United Healthcare CEO wasn't exactly high profile. I doubt if many people outside of his immediate circle even knew who he was or could recognize him. So how is he supposed to "rethink his lifestyle?" Guaranteed some business is going to piss someone off.
