My mum has been scratching at her tattoo for a while because lumps/ sports have started to pop up and now it looks a right mess. I did message the artist who done it but he not got back to me and mine has started doing same thing. My mum has many tattoos and they have never done this before

A month is a long time. It seems like a reaction to the ink, but nobody here is likely going to be able to tell you for sure. What you do not want is a simple skin reaction to get worse and affect your mother's entire system. This could be a simple rash or this could be an infection that could go septic.
She should be able to walk into an urgent care clinic or see her doctor without much trouble. Regardless of the trouble or price, it will give both of you peace of mind and keep her safe to see a professional who can properly diagnose and treat her. Good luck.
