It’s out of autonomy. The patient can decide if they want medical treatment and they can refuse it.
They can even be discharged against medical advice.

Hun, when they’re unconcious, they’re unable to give out conset. That being, medical personnel must 1) do no harm 2) beneficence. If they have no DNR status, it is their duty to save their lives. God, big pharma and saving lives! There they go again. It sucks when they do that @Peaceful
