When I tore my tendon in my left arm I was going to physical therapy and they would put two pads on or around that area and then would set the intensity on the TENS unit.
After I had my surgery I was in a soft cast for 6 weeks or something, and when that came off my arm was frozen... I wasn't able to bend my elbow so had to do more physical therapy for that, again with the TENS unit.
That's been my only experience with TENS units, I don't know how it would work with someone who has neuropathy though.
I would definitely consult with a doctor for their recommendation.
