They can be fat before developing anorexia or because they're starving themself, their body will convert the few things they do eat directly into fat so they only make it worse for themselves.

Anorexia is a general loss of appetite, or a loss of interest in food.
Anorexia nervosa is a serious mental illness. Patients have not "lost" interest in food, they have intentionally restricted their food intake because of an irrational fear of being or becoming fat.

Yes. Anorexia, basically, is loss of appetite for food. It can affect anyone. A fat person with anorexia may be hard to recognize, though.
