Just sincerely apologize, you've had a rough few days and we're stressed about that other situation, tell her that you feel crappy about forgetting it and it wasn't your intention at all.
After the dinner surprise her (if your budget will allow it) with a couples massage and and a romantic evening. Totally focused on her!

Certainly the best answer! If you can convince her that there was some justification in forgetting it's your best option.
