not only are the effects of this on RNA expression [detrimental? whatever it does or is it supposed to be cytotoxic?], but ...
I think I need to understand better what you want to say.

@malcontent: no, the [i]not only...but also...[/i] phrase is the subject here. idk if [i]is/are[/i] is used here

@cranberries: the only thing I see wrong with this sentence is that it needs two commas; one after expression, and one more after itself. There should probable be one right before itself, too, but I'm not sure it's really necessary.
