Ever have Soup Explode in the Microwave?

I was heating up a bowl of clam chowder in the micro, covered with a paper plate, like I've done more times than I can count. Suddenly it goes *POOF* popping off the plate and spewing clam chowder all over the inside of the microwave. After cleaning up the mess I heated what was left in a kettle on the stove. WTF?

You have to stir thick things halfway through the cycle.
@HollyW I actually did. I always heat up in increments so things are heated uniform. It was my second cycle when it went nuclear.

@BrewCityBarfly Use an overturned plate instead of paper.
@HollyW I think if I heat up soup in the micro again, will use one of my micro bowls with the vapor hole. Also need a vented plate cover, not sure what happened to mine.
