Surely at least one child could communicate what had happened to the injured boy??? I mean, someone told you what happened.
The injury report would have been ideal - but, even in the absence of the report, it shouldn't prevent the nurse from doing his job.
Let's be fair here, most school staff are only able to provide first aid until an ambulance arrives - and the nurse treated the injury as best as he could under the circumstances, since he determined the injury to not be severe enough to warrant the child's removal to hospital.

@HootyTheNightOwl One of the school staff was a bit ansty because apparenty they didn't get enough information ( the injury report) to find the student information to inform the parents. If only the nurse would of asked what is your teachers' name then they would of known what grade they are in both students know their own name because if they can say that they are in kindergarden which they were mistaken they are in pre kindergarden they should of asked who is your teacher and what is their first name ...then intercom the classroom to ask me to come into the office...while the substitute tends to the students(even though I felt uncomfortable leaving a old lady with the students).
@OpalFlower Surely getting the child's details isn't that hard???
Makes sense to me that their name and age should be all that's required - or ask for the teacher who sent them, like you said.
If the issue was just a form, then you could have filled it in without leaving the room and just sent it to the nurse using the substitute teacher... that way she's not left to deal with your students while you take care of the matter at hand.
You never know, if they'd asked for the form then you might have had it done and ready to go by the time that she arrived in your room.

@HootyTheNightOwl Well one student spoke Spanish and knows somewhat English. The other student knows English. Even with that said they easily know their teacher's names. And all the students know one another's first names.
And right I could have filled it in without leaving the room and just sent it to the nurse by the substitute but it slipped my mind since I was tending to the other students.
Now if they intercom me saying that they needed the student information...I would of written one up and apologized to the substitute telling her that I forgot and would ask her if she can take the form/information to the nurse office. Even when the substitute came back saying the nurse sent her back because he was on the phone....I didn't even remember the injury report still. I guess today is forgetful Friday then Mistake Monday lol jk. I am somewhat thankful for this experience because now I won't forget.
