It came from the deep south in the 19th century, pre-dating the Civil War.
The title may have referred to someone who was drunk, or blind, or had glaucoma, making the eyes appear very light and possibly clouded.....so the eyes looked like cotton.
The first written version of the song appeared in 1882, and the first recording was around 1923.
