My first car did. It didn't fail because it was killing the environment, it failed because their computer wouldn't establish a connection to my car. I replaced my car's obd2 port and computer, still no go. I spent enough to get a cost waiver for that year. Two years later they expected me to spend a small fortune again.

Long,long time ago when i lived in NJ but here in Florida it's a joke. I've seen vehicles here belching so much smoke,they could've worked for the mosquito control commission.
Ya there was a time I failed because the car hadn't been driven enough for the computer to clear all the codes

Yes, multiple times. Had it turned up and it still didn't pass. Finally got rid of it.

On a diesel yes. Just added a can of forte in the tank. Revved the ass out of it in 3rd for a bit and cleared out all the crap. Emissions passed. But that was a few years back.
