How can my "ugly" friend improve his appearance to have better success with women?

Before you say anything, yes, I did have clear and explicit permission from him to use this photo, and it was him in fact that told me to use his picture online to see what advice I could get for him.

But he's really insecure about being "ugly." And honeslty, despite beauty being in the eye of the beholder and everything, I unfortunately don't disagree. He's bald with a big red nose, and you may think he looks old but believe it or not he's only 34 years old. He just has a history of taking zero care of his body since childhood (including neglectful parents) and doing tons of drugs, eating tons of junk food, terrible hygiene, and never exercising.

The worst part though for me is his personality. He practically does nothing productive with his time. He claims to be "temporarily unemployed" but he's been unemployed for over 5 years now. He also keeps posting his selfies all over 4chan, and people on there have been making fun of him for over a decade and yet he keeps on posting. And he's terrible at talking to girls as well. He literally starts all conversations with women with "heyyyyy prettyy gurrrlll! I'm Ceciiiiil!"

But he refuses to change his personality and wants to work only on his appearance. And so here I am making this post asking you guys for advice.

@Husher Excellent advice! I'll share all of this with him. Hopefully he actually listens for once.

And yes, he certainly has way more problems. He's 34 but acts like some sort of immature 14 year old who refuses help from anyone.

@Husher Sigh...well I tried. I fucked tried. I kind of expected him to ignore my advice anyway, but it's always disappointing nonetheless. Even when he explicitly asks for advice he just ends up ignoring it or making up excuses why he can't take the advice.

Worst thing is he never listens to neither people younger than him OR older than him, and not even people the same age as him. At first I thought it was because a 34 year old didn't wanna listen to someone in their 20s, but he doesn't listen to older people either, including his own parents.
