I feel dumb for asking but so many people on TV have insane hourglass proportions...how do they acheive it?

Is there a surgery for hip widening? Is it waist training? Is it a type of excercise to build the curve?
It's not their hips it's probably their Brazilian butt lifts where they transfer fat to their bottom giving the illusion of wider hips.

They are perfect because they won the beauty contest first and got on TV afterwards. Who else would qualify?

It's largely just selection. Plenty of human variety out there, and what is fashionable gets overrepresented in media.
