I just made a huge blunder on my work, what do you do if you are me?

I just made a huge blunder on my work, i just recruited for about 1 month in this my new office and due of my colleagues (who usually make MoM ) was taking leave I was assigned to note minutes of meetings. I made a blunder noting wrong information, and the MoM is already circulated as an official MoM.

I was sending the MoM draft to my work colleague to be corrected or reviewed, but looks like she not actually read all the MoM...

Ah... The matter become a big fuss on my department.

I justified my self that I already sent the draft for reviewing. But it just feel like a cheap excuse for me.

Argh if you were me, what would you do?

Should I also make an apology statement for the mistakes?

@blackravencat calling it a correction kind of implies the 'sorry" unless you know of someone who used the incorrect info specifically.
