How does one ask for feedback from a former employer?

You just ask your former manager.A lot of employers do exit interviews now.So when you are leaving you can ask for feedback.
You could phone/email a former employer and ask for feedback.

@tallpowerhouseblonde Thank you for the advice. I wasn't able to get a hold of him, but left a message for him asking for feedback. We'll see what he says if anything at all, but it should work out either way.
