Yes, many times when diving into swimming pools and swimming in the ocean when I didn't notice that my top had come off.
And it wasn't always just guys that saw it.
One time I dived off a charter boat filled with people in the Molokini Crater in Maui, Hawaii. The boat has a large hydraulic ramp that lowers into the water so people can actually walk on and off the boat into the water easily.
When I was coming up the ramp my sisters started yelling at me that my top was around my waist. A large number of the people on the boat were watching.
I was raised as a nudist so I didn't really care and no one seemed offended by it anyway actually.
