I was about to go to bed, but I see you are serious and we should have this talk.
The best hot chocolate is made with chocolate - quality really matters here so get good stuff. 70% cocoa content is about right, but let your tastes guide you.
Grate, shave, or chop the chocolate so it's small and melty. 1-2 ounces worth.
Heat up a cup of milk (whatever milk tastes good to you I don't care). Whisk in 1-2 tablespoons sugar, a pinch of salt, and one tablespoon cocoa powder.
Then remove from heat, and add the chocolate and whisk in with gusto until it's all combined. That's it.
Mix in whatever you want. Booze, spice, throw a marshmallow own, etc. Skip the cocoa and add more chocolate if you want it even richer. 3oz for a cup is probably the ceiling for this.

@SailorMarz The other folks posting about fancy cocoa powder. Get that stuff because it's amazing. But if you THEN get a really good chocolate bar and grate it in you'll be in heaven.
Or shit, some grocery store milk with a bar of lindt and some Fry's cocoa powder will still make something pretty great.
