Apologize and move on. If you both have said sorry and accepted the others apology, forget about it :)

A big blowup is the thunder and lightning after days - weeks - months - of threatening skies. It should clear the air.
Another thing to consider is the major stresses you and - as someone who loves you - your sister have been under with your health challenges. As my husband and I learned during his long illness, you can only be THAT nice to someone for so long.
The awkwardness will ease - probably as soon as you two share a hearty laugh.

Try to push past the awkwardness. You both have apologized, so it's time to move on. Don't allow it to continue to come between you. I've found laughter to be the best easement of post-apology awkwardness. Find something to laugh together about. Or something to help her with.

Maybe go out and buy ice cream and casually ask if she wants some too? Or any restaurant for food. Food often solves my problems lol
Edit: Oh and yes, I do feel awkward but I try to pretend nothing ever happened and forget about it

You'll get over it

yeah I think right after big fights, things can be a bit awkward for a while, but before you know it things get back to normal
Sometimes that happens when there are residual emotions.
