How do movie makers approach an unattractive actor when they want them to play someone ugly?

Like this? "Hey! We're making this movie and we still need someone to play the ugly freak and we think you would be perfect for this role."

I was offered a role in a movie. I said no thanks, I had no interest in acting, but then I had to ask if it was a bad guy part. I was told, "In fact it is the bad guy." A psychopath drunken homeless dude. So much for my my suave and sophisticated good looks!
