how do I leave SW, for real?

I've tried many times and answer their questions, but I don't see any way confirm or complete the process. If I click on a reason, it gives me a response and then carries on as if nothing happened. Even if I click "never" for when I want to reactivate, there 's still no way I can see to complete the process.
