Your short-hand for what happened is pretty bent.
The gun wrangler was high, inexperienced, and unorganized, and not following best practices for a position where ammunition and guns are involved. She was an undependable team member.
Had she been part of the sound crew, they might have only lost a few takes due to a wrong button being pushed or not pushed, there would perhaps be a factoid for trivia for that film.
Unfortunately, she was the central figure in why a person was killed who should never have been in danger from a slug being shot from a gun on the live set.
