When someone says they are spiritual, what does that make you think of?

Crazy. It makes me think they are most likely crazy. They are trying to fill a hole in themselves that doesn't exist with something else that doesn't exist. That isn't to say that I'm against spirituality, quite the opposite, I think it is very important and I think all individual spiritualities are valid, even the crazy. But, it is my experience that the majority of people claiming to be spiritual are idiots.
They don't know what they are talking about, they don't investigate or examine their spiritual teachings or teachers, they are just giving money to some fake guru because it makes them feel better, they imagine things that aren't real because they think it makes them different or interesting when it doesn't, they are delusional etc.
You asked me. That's exactly what I think, and more often than not I'm completely accurate.
Examples: "I saw Jesus and he spoke to me." "We are one with the universe." "All paths lead to God (or Truth, or Peace)." "Buy these flowers." "Donate to God's cause." "I believe in evolution." "Get vaccinated." "Vote democrat (or republican)." etc.
Spirituality is not some phony bullshit. Spirituality is everything.
