About 10 years ago my wife told me she had lost interest in sex, and I have gone without for a decade now. The situation is such that if I want even a kiss or hug, I have to ask or initiate the process. I am at my witts end. What can I do? What should I do?

Your question is the answer. I am going to assume you have tried to discuss and change this, so that option is gone. Thus you have to decide to stay and accept it or go regardless of the cost to other parts of your life.
