In a Excel/spreadsheet file:

1. The file is password protected, this password I have memorised.
2. Every tab/sheet is meant for a particular type of accounts like, Email, Social(Networking), Career, Office(Work), Cards, Banking, Shopping, etc.
3. Every sheet contains columns like, Website, username, password, email, recovery detail, description, other info.
4. I keep this file in my google docs. I have a google drive app on my Laptop that keeps a copy on google server synchronised with the copy on my laptop folder. This prepares a backup in case of your system crash.
5. Google can't open the file on browser in edit mode. And also requires you to enter the file password before you can even view it online. This makes the information accessible remotely in hour of need, but securely.

More Points to take care about..

6. Always secure your accounts with good passwords and two factor authentication whenever required and available.
7. Always ensure that you have configured the recovery options securely and the information is up to date, like your current mobile no. Permanent email, etc.
8. Don't use public computers to access sensitive information/website. Even if on a reliable person's system, use incognito window.
