A leash and harness, yes. They aren’t allowed to go out on their own. The one pictured in the harness is actually mentally disabled, and his sister has never been outside on her own (she doesn’t like it). My other two were raised in the country with free reign - now I live in town on a busy street. I don’t think they would last long on their own outside. Fortunately, they don’t usually want to go out.

Normally, cats want to go out if (or especially) if you keep them inside all the time.

He got sick and his brain was more or less fried. He’s been off ever since.
Mine only occasionally want out, and I try to take them outside when I can. I’d rather keep them inside where they’re safe than find them on the side of the road on my way to class.
