Online disciplinarian

This may sound odd. The reason I am looking for a disciplinarian is to take away the option. Its too easy for me to not do it even if I deserve it. I want to tell someone my behavior and they make the decision on a punishment. Here are the habits I am trying to correct. The biggest one is smoking cigarettes, gambling (nothing crazy like thousands) exercising more frequently and not being so lazy when I'm home
