It is utilized as an insult but I cannot comprehend why.

Here in the North Eastern US it's not too bad in most places but there are still factions in our Country like the far Right and the Christian Nationalists who are trying to go back to the way it was in the 50's and are dead set against same sex marriage. Some kids still use it as a slur but they too are slowly progressing...
