The Expression "Sam Hell"

I have heard the expression "What in the Sam Hell"; are you doing, is going on here, etc. Who is Sam Hell, and why is he associated with people doing something they shouldn't? :)

Sam Hill is a euphemism, of unknown origin, for the devil, so it’s like saying, “What in the devil (hell) is going on here?” so you were close.
