As long as the middle man/middle system does not get too much out of it.
Doing it for free should be a choice not an obligation.
It is funny that you will have to pay for the surgery because doctors have financial needs to fulfill , same for the hospital. And they get called heroes.
But the guy who actually was there on the table risking their life gets nothing and it would be selfish of them to ask.
