Americans say "in the hospital". Most others say "in hospital".

I change it up depending on who I'm talking to because I'm cultured and fancy like that.

An aside: Russians don't have to puzzle with this question. As I'm told, their language doesn't contain the definite article "the." That's why it sounds strange to hear them say in English, "here is house", "I go in building", "there is Brighton Beach."

We say "in school", "in church", "in town", "in jail", so "in the hospital" makes us inconsistent.
