Even if everyone followed all the instructions, the availability of the testing, the lack of PPE, the need to keep working to survive and not shutting down public transit pretty much guarantee it will spread. Asymptomatic/mildly symptomatic people still have to use buses and trains to go to work and shopping, the common spaces are not being cleaned after each use so one person on a train infects hordes more. People with symptoms struggle to get tested, so communities don't know the real figures or risks, and very few people can afford to skip work anyway so they'll have to go out everyday and infect more people. Or they'll go to a hospital and infect a healthcare worker who doesn't have enough protection, and that healthcare worker infects all the patients they interact with.
@plungesponge As I understand it the idea is to not overwhelm limited emergency services.

oh absolutely. The issue is that this wasn't prepared for at a national level early enough and even now the reliance on individuals and state governments to manage it isn't going to be enough to keep the numbers down. Unfortunately, this is going to end when the virus runs out of hosts, not when the government gets control of it
