Like do not resuscitate but stating not to take one to a hospital

@Jackaloftheazuresand I can't remember exactly, had if filled out mid last year and has been in a file since. Refused treatment or something like that. I'll have to look in the file. Patient has to be terminal to have it though, which my wife is.
