Sometimes it's genuine, sometimes it's a passing fling, and many times it's for publicity. Hollywood romance and arranged marriages aren't all that different.

I would assume it happens just like when people meet in any other situation, like at any other job. They meet, talk, hang out together, then date, fall in love, just like anyone else!
