Always. I learned from my mother. I get my groceries delivered by Peapod now, but I still write a list and follow it online. When the driver delivers, I give him the coupons and sign for its receipt. After he goes, I check it to make sure it is correct as I put everything away. If anything is missing (it happens) I call the office and get credit for what it is.

If I'm going to Walgreens, or CVS, or somewhere else, I have a list for them, too - and maybe a coupon or two.

Yes and sometimes forget to take it or lose It halfway round the shop which I haven't got a clue how i do that.
Then I never remember to add things I couldn't get to a new list.

Yes I do, hopefully don’t misplace it before I get to the stores. It makes shopping trips a lot quicker if I write out what I want to buy at each store.

Yes. Whether I forget to bring it with me, or not, I often manage to forget to pick up at least one thing on it.
