I think it has to do with how much it affects your behavior. Alcohol does have stigma around it, but it has been around for thousands of years, and so most of society has kind of accepted it culturally as part of human life.
Cigarettes, more than it being legal, I think doesn't have as much stigma as alcohol or other drugs, because it doesn't really change your behavior with others through its effects. As in, cigarettes are condemned because they are unhealthy mostly just for the person who uses them... and it is seen with a certain level of contempt by some people these days, who consider it a bad choice.
However, you would be less worried about a person smoking cigarettes as far as them harming you goes or them making bad life decisions because of the cigarettes. Its effects are mostly unseen except within the body.
Whereas the other drugs have more stigma because people believe, either correctly or wrongly depending on which drug, that a person under the effect of those drugs might be dangerous to others or have loose morals in life. Some of these beliefs are based on reality, but some of it has been reinforced in most people by societal powers for a long time.
For example, I have smoked weed and I know for a fact that it doesn't really make you dangerous. It makes you more chilled out or slightly weird, if anything. But where I live it is still grouped with hard drugs like heroin or cocaine, legally and conceptually, and most people here still believe smoking weed makes you prone to violence, and that it is something 'criminal elements' do. The attitude has been slightly changing because it's becoming legal in the west... and the conversation is better now, but I don't see the laws changing for a long time anyway.

Then so are coffee drinkers. Usually people are considered to have a drug problem when it affects them socially.

@Flenflyys Yea just how it should be. Almost everyone IS a drug user because they are. Just because someone doesn’t do it excessively doesn’t mean they’re not considered drug users. They USE drugs therefore they are drug users.

I don't think legality has anything to do with it. It's about money. Cigarettes are some of the most cancerous things you can smoke, but they sure do turn a good profit. So of course they won't want people looking down on them.

So would alcohol. Anything can be considered a drug. You can be addicted to anything, food, sports, video games, porn, etc. But yes, cigarettes are a drug.
