Name, DoB, and blood type over my heart. That sort of information is crucial if you're being treated while unconscious.

@Red30 Err... Sorry if I stepped on your toes. And yeah, it's a good precaution. I'm kind of leery of tats in general, though, after a friend of mine got an infection.
