It depends on the person and situation. If I can, I do it in person, make eye contact, and make sure they know that I know I hurt them and I'm sorry and how I plan to fix it. If I can't do it in person, the next best is over the phone. Last resort is in writing. The biggest thing is to be honest, own what I did and NOT make excuses or say "I'm sorry but....." If the other person hurt me, I deal with that at another time so my apology doesn't get lost.
