What causes the most harm in the world, but is completely avoidable?

quick easy answer is tobacco. But please, give it another 50 years, I like a fine cigar with my whiskey, and if my cousin couldn't light a cigarette while driving and dealing with the idiot drivers in southern Florida I fear he would kill many of those a-holes.
