How do you go about asking someone to be friends with benefits?

I don't want someone to take it the wrong way or ask the wrong person. I've literally never done it before so I dont know where to look

You have to complete a long winded form of 15 pages with details of your past history, your current income and exactly why you want to apply.
Be warned, there is a 1 year backlog of applications.
