To those who speak a second language

My Spanish level is high intermediate. I'm embarrassed to say that even though I've studied off and on for years I cannot say that I'm fluent. I can read anything easily. I've tried so hard but I just cannot understand spoken words unless I concentrate so hard that my head hurts or only when its spoken rather slowly and deliberately.
When did the point come for you that you could totally understand your second language at normal pace without having to think about the words?
Is anymore progress possible at my age?

Go live in another country? My high school Spanish got a big boost. But now it is shrinking again. Panama. Affordable.

@FlowersNButterflies Yea that has been my ultimate goal now for a long while but I keep having to put it off. I actually have a rental property about 5 miles from the border with Mexico and I do get to use my Spanish occasionally but last week a woman in one of the border towns could not understand something that I tried to say to her and I got frustrated with myself.She could not speak English so it was really embarrassing to study that long at then fail when I needed it. I felt so confident right before that encounter.
