Also, what do you mean there's no such thing as ice in Europe? I live in Europe and I assure you there is ice, including in our drinks.
And by the way, we get free water here in the UK in any restaurant, unlimited refills.

What do you mean by "no such thing as ice"?
As for "free" refills, I rather doubt they exist anywhere. I reckon any cafeteria can offer such either by gambling on loss-leading, or by simply increasing the base price slightly to cover the possibility of customers requesting refills.
I have never vsitited the country but I am sure there are plenty of good-quality restaurants in America serving excellent food.
Sadly, that is not the image American big-business gives to the rest of the world, outside of very expensive hotels anyway.
Instead is either of huge US companies building endless numbers of cloned outlets for high-price-poor-value junk-food in everyone else's countries. Or it is as the films suggest are ubiquitous within America, the down-at-heel, roadside greasy-spoons frequented by clinically-obese bikers and lorry-drivers with a penchant for malnutrition.
So if European tourists fed such stereotypes by US business, visit the USA and find the decent restaurants, they can be forgiven for being surprised.
