In my city, it's becoming more common to add a 20% service charge, that gets divided among the service/kitchen staff. Otherwise, it depends on the service, starting at 15%.
Last weekend, I was having lunch with a date. She had to ask for tea a half dozen times, the waiter got my order wrong. I asked for a prawn salad, he brought a chicken dish. We waited 45 minutes for our food, and when the bill came, he had charged me for the chicken dish on top of the prawn salad. I did not want a dessert, but he dropped a complementary one (for our trouble), that neither of us wanted, only to find that I was charged for it anyway. I did not see it until after he ran my card. When I complained, he said they will fix it later.
Even with all that, I felt compelled to tip him 15%. I worked my way through college, doing all sorts of jobs, including waiting tables.
