Have you ever looked at the "End of life planning" forms?

I swear they ask the same thing five different times.

This one really made me think:

During the last days of your life, what's most important to you (for example, spending time with loved ones, dying at home, spiritual observances, listening to music)?

Any thoughts on what you would put? Or is that too personal?

When I was in the hospital having my last I had to fill out a bunch of paperwork, including what I would want to happen in the event that decisions needed to be made. It was a real opener for my husband and myself that we really needed to have a serious conversation about choosing to stop growing our family. Filling out all the forms took a major toll on my heart and soul as I was already in the middle of a very complicated labor.

- Lord Huron (can be repeated)
- An audio book version of the Hitchhiker's guide to the galaxy
