Go to her before she pass's if she does ....
Tell her how you feel apologise ...hold her hand whisper you love her ...even if she doesn't respond she will remember how nice and loving you were to her in your younger years ......
But don't blame yourself for what you said before ....we never know what's round the corner ....your gran will know in her heart and dreams ...You LOVE HER !!
All the very best

@Bobsy2018 I sincerely thank you

I, too, am persuaded they hear us, even if they don't or can't acknowledge. Just so you don't feel like the Lone Ranger, I had a similar experience when I was about your age. Here's part of a bad poem I wrote in the hospital room.

That burning word that trembles on my lips
That hateful thing I've got to say
About another's act, or lack.
Just stop and think before I let it go:
What if I never get the chance
To take it back?

Tell her, she'll hear. And she already knows.

"Unspoken words are seldom regretted" - Author unknown
Go now before she worsens and tell her from your heart how you feel. No regrets. Words spoken can be lessened. But do not wait. Do it now. She loves you. You love her. This is happening to her. Make haste.

She is hearing this. Don't beat yourself up. Keep sending her the love you want her to receive.
