I think I had the opposite experience the first time someone shortened my name. I actually liked it and it stuck.
I can see how this would normally be annoying though. My last name is often mispronounced, even after saying it more than once. It's super easy to pronounce too. Go figure.

@Piper I think the point is, is that you can tell what someone is about.
I think of my own real name and how some kids used a shortened version of it to tease me. Then there were others that could call me the same thing and I knew it was a positive thing.
Having good intuition about people really helps in such a situation. People will always get our names wrong, but what goes behind it is really the issue.

@Peapod I think I even asked you, if you minded me calling you Peas. While this question was 'inspired' by a conversation I had on the phone with someone I'd never spoken to before and probably never will again...it is still a similar sort of thing. There is no hard and fast "rule", about when it can be a little annoying.

It is that and other times like that spurred this question, and not just directly related to [i]me[/i]. I did not admonish the person, and there was no playful, "teasing" term of endearment in any way possible.
