Do you know what the name of this movie might be?

So I'll try to keep it short, but please bear with me. I have memory loss due to seizures. So, I don't remember all aspects. Things I remember, I could be remembering wrong. Just wanted to put that out there for this question.

I've only seen this movie twice. I want to say it was in black and white, but I don't know for certain if it was. I think it was a movie on Twilight Zone. A married man is working, maybe on the docks, like boat docks. And a friend? Or someone he knows offers him work? I remember something about if he goes through a door, he can't go back and that they were able to make it look like he died, but was missing, and that if he went through the door, he couldn't go back to his wife.

Then I remember he was in this room, where there were a bunch of people at desks typing, and there's a single desk at the end of the middle aisle where someone was sitting that he talks to, about finding different work. At the end of the movie he's strapped down to a gurney and has a lobotomy. I don't remember anything else about it, just those main things.

Any idea what the name of this might be? Thanks in advance if you can help out, I've been looking for it forever, but I just can't find it.
