When I was working, all of that. My position was kind of unique though.
Vest, gloves, safey glasses, steel toed boots were mandatory all the time. Hard hat sometimes.
Masks were not always mandatory, yet if you didn't... well there was a lot of dirt in the air.
Respirators were a necessary evil. Yet if you are working around hazmat in an enclosed container you better use it.
Harness were rarely used. Yet one time I had to use one while looking at a second story air conditioner. Pictures were wanted.
One more piece of PPE.
Working around hazmat many times the white suit was necessary, with latex gloves.

Long ago I was installing insulation on the third floor at CAT factory, new extension to it really. And because the lift was leaning slightly outward I just couldn't reach the wall. Harnesses just weren't available and I had to get outside the cage to install that whole section.
That was intense. I was the only one that would do that. All the other workers were put off the job before the job was completed. So I was the last person to complete it.
