I do call my Pastor Pastor Pena because he earned it. But he does not go around expecting people to refer to him as a Pastor or reverend because he is a very humble man.

No, professionally speaking if you will use such titles in a formal or public writing you'll get lawsuit! It is a form of deceit that can lead to fraud. Caveat.
